Step 1. 

Complete the Member Application Form and:

Step 2.

When we receive your application, a Care Team Member will call you to schedule a friendly home visit.
                We like to personally meet our members so you can tell us about your preferences, interests, talents and needs.

Step 3.

Following the home visit you will receive a call telling you about the next steps.

Step 4.

If a membership is offered, payment options will be discussed. We enter your membership information in our secure computer system.

Once the above steps are completed, you will be invited to attend a New Member Orientation and become well informed with all the services your CVN membership offers. You will receive an online CVN Membership Handbook which outlines all our procedures and services. Additionally, you receive a personal membership number allowing you access to member only information on the CVN website.
